Applications:
DSACF Scholarship Application (PDF) This form is the application high school seniors should use to apply for most Duluth Superior Area Community Foundation scholarships. This is the form sometimes referred to as the Common Application.

Minnesota Power Community Involvement Scholarship  Students should visit the Minnesota Power Web site to learn more and start an application for the Community Involvement Scholarship.

Minnesota Power New Generation Scholarship  Students should visit the Minnesota Power Web site to learn more and start an application for the New Generation Scholarship.

Greg Irons Student Application (PDF)  Students should use this form to apply for the Greg Irons Student Scholarship. 

Greg Irons Staff Nomination Form (PDF)  People interested in nominating an employee (faculty or staff) of the Duluth Public Schools for the Greg Irons Staff Award should use this form.


College Scholarships Application Form (PDF)  College students interested in applying for the Darrell & Palachie Asselin, Modern Woodment of America and Julie Enberg Memorial Scholarship should use this form.

Lake Superior Medical Society Tilderquist Scholarship Application (PDF)  Students interested in applying for the Lake Superior Medical Society Tilderquist Scholarship should use this form.

Small Business Education Scholarship Application (PDF)  Business owners and entrepreneurs interested in applying for the Small Business Education Scholarship should use this application.

G. Scott Ransom Scholarship Application (PDF)  Athletes interested in learning more about the G. Scott Ransom Scholarship can visit the Duluth Alpine Club Web site to learn more and download an application. The deadline to apply is September 20 every year.


Forms:
DSACF Scholarship Recommendation Form (PDF)  Teachers, counselors, advisors and administrators should use this form to recommend a student for a particular scholarship.

DSACF Transcript Release (PDF): This form can be used by Duluth and Hermantown students to request a transcript from their school be sent to the Duluth Superior Area Community Foundation for academic review.

Recipient Forms:
Acceptance Form (PDF) Students who have been newly awarded a scholarship should use this form to accept/decline their award.

Renewal Form (PDF) Students who would like to renew a multiple year scholarship should use this form to apply for a continuation of their award.

Scholarship Final Followup Form (PDF) Students who are graduating and otherwise finishing their scholarship should use this form to report back to the Community Foundation about how they made use of their award.

 

NOTE ABOUT FORMS

The above PDF forms are designed to be completed and saved with Adobe Reader. Most of them can be opened with web browsers (Internet Explorer, Firefox, (or for Macs, Preview) etc), HOWEVER, by doing this, the save function may not work correctly. It is therefore important to use Reader to create a document which is fill-able, save-able and return-able. The link to download the newest version of Adobe Reader is here.

Once you have the correct program, save as/download the form to your desktop; open the Reader application and then open the document with the 'Open' command under the 'File' dropdown.
